Memory Care

The Heartlands is proud to provide memory care for residents with forgetfulness, Alzheimer’s disease, and related dementias. Our memory care community is a safe and secure environment featuring 12 spacious, private apartments each with accommodations and technology to meet the needs of each resident.

• 24-hour care and support as needed based on individualized care plans.
• Assistance with activities of daily living including dressing, bathing, and transportation to meals/activities..
• Medication management
• Three delicious meals per day.
• Daily housekeeping and laundry services.
• Emergency pull stations in all apartments.
• Daily life enrichment programming designed for those with memory issues facilitated by specially trained caregivers.
